You will need a dedicated server, perhaps more than one with
load-balancing if you are running lots of server-side activity.

Apache should be handle to the work load, despite claims that it cant,
and I'd suggest the Unix family operating system for this number of
concurrent connections, it provides good tools for monitoring loading
and the like.

Yes, same price at servermatrix.com and ev1servers.net

These are starting prices, I suppose it depends on his requirements.

Quite. It's worth mentioning, for the benefit of the less experienced,
that when looking for hosting one does have to consider individual
requirements, for example:

1) Disk space
2) Monthly transfer allowance
3) Type of support
4) Scalability
5) Terms of service

Which host is right for any particular site will vary. There is a
host, for example, which will happily host terrorist web pages,
racism, criminal activities and the like. Others will not even host an
exposed nipple. It's their choice, but it's worth carefully reviewing
the ToS and if in doubt asking the hosting company explicit questions
before signing up.

I missed the original, but we certainly satisfy in excess of 4 million
visitors every month, no problem with tera-byte.

The server often (read frequently everyday) has 500 or even 600 or
more concurrent connections open.

Hmm... there were two providers mentioned, not just Affordable Host. While
their pricing isn't the cheapest I would hardly say they're expensive. And
often the cheapest provider of a service isn't the best. And I'm not saying
they're the best; I have no experience with any US based dedicated server
providers other than Colossus from a few years back, and they're REALLY
expensive.
