I have an ISAPI extenion application. This works when
protection level in IIS is set to medium in most of the
machine. I ave noticced that it requires to set protection
setting to low in IIS in a few machine. Please help.

Okay!

You dont want to tell us what actually happens when you set it to the medium
level and it fails? Error messages? In the event log?

It is quite difficult to say anything without any information about the
error. That would only be guesses, which usually ends up in more work than
need to be done.
