Fastclick are excellent, it's official, with banner eCPMs of about
$0.50 (and much more).

Casale are also very good with banner eCPMs of about $0.30

For an excellent comparison of advertising agencies, from a web site
publisher's perspective, take a look at:

I'm not sure I agree with their rating for Google adsense, returns
seem to vary widely depending upon the publisher's site.

How much do they charge the advertisers though?

The publisher only gets a miniscule percentage of this.
