Here are the steps as listed in the forum at

Because the configuration data formats of the EP-4501/EP-4504/EP-4508
broadband router models are somewhat different between each different
firmware versions, it is necessary and better for you to restore the
configuration data back to its factory default settings after each firmware
upgraded.
If you do the configuration restore just for lose of the user name and
password, then maybe check to use the default user name as admin with
password surecom in all lowercase letters.
For old hardware reversions before 1D (that is, 1A, 1B, and 1C) or any new
reversions:
1. Follow the same steps for Hyperterm connection to the device as
described in the firmware upgrade document on our download center or in the
user's manual.
2. In the console mode, press h key for commands help.
3. Press the r key to reset the device back to factory default.
For new hardware reversion 1D and after with hardware reset button:
1. Remove all connections and Unplug the power of the device.
2. Press down the reset button still until the step 4.
3. Plug the power back to the device.
4. After the LEDs signal that the reset has been done (the LEDs are not
blinking around), let go of the reset button now.
5. Unplug and then plug the power plug back to the device again to reset
the power of the device.
Now, the device is restored back to factory default and you could configure
the device again for your Internet connection and application.
