the only one that I think have meaning for this is - content expiration is
set to 'expire immediately'
The rest is not in your hands - There can be proxy between user and your
server or just dump user browser.
try from the browser to hist Ctrl+F5 to make clean refresh. And set to check
for new version of page on every visit.
