Thanks to Jeff and Roland for answering.
Turns out it was something very simple but I still do not know what exactly
it has to do with this issue on my IIS Server.
I had File Expiration set on this server to "Expire Immediately". Once I
turned off expiration it worked. When I set Expiration to "1 Minute", it
still worked.
I have no idea why this is the case. We had Expiration set so that any new
file will always DL'd to the Browser. We often have HTML or PDf reports
posted to the Intranet in rapid succession, sometimes some reports get
changed several times per day. We had had issues with people pulling the
same version of the report for days and basing decisions off those old or
incorrect numbers until we set Expiration on the Server. Setting Browser
settings to check on each visit made no difference.
So who knows why Expiration caused this? I don't.
