use a premade script:

<a style='text-decoration: underline;' href="http://www.hotscripts.com/PHP/Scripts_and_Programs/Form_Processors/" target="_blank">http://www.hotscripts.com/PHP/Scripts_and_Programs/Form_Processors/</a>
<a style='text-decoration: underline;' href="http://www.hotscripts.com/Perl/Scripts_and_Programs/Form_Processors/" target="_blank">http://www.hotscripts.com/Perl/Scripts_and_Programs/Form_Processors/</a>
<a style='text-decoration: underline;' href="http://www.hotscripts.com/ASP/Scripts_and_Components/Form_Processors/" target="_blank">http://www.hotscripts.com/ASP/Scripts_and_Components/Form_Processors/</a>
remotely hosted
<a style='text-decoration: underline;' href="http://www.hotscripts.com/Remotely_Hosted/Form_Processors/" target="_blank">http://www.hotscripts.com/Remotely_Hosted/Form_Processors/</a>

when you find a script you like do a google to make sure it hasn't got
any security issues.

There is no such thing as an email form.

Some people claim that there are such things as email forms but they are
misguided, often dillusional and always forgetting of the technical
issues involved.

I see you have already used Bravenet. Use one of their services to make
a form that is processed by a server-side script. This script can then
send you the form information as an email, and it is more reliable.

Other options will require a better host than freeservers. I recommend
you get a decent host (there are plenty regularly mentioned here that
are quite good; if you are in the US or Canada you could check out
<a style='text-decoration: underline;' href="http://www.1and1.com" target="_blank">http://www.1and1.com</a>). You will then be able to use a pre-made script
(freeservers does not support any server-side scripting) which does the
basically same thing as bravenet but which is tailored more to your
needs and doesn't have a silly logo on it. There are plenty of such
scripts at <a style='text-decoration: underline;' href="http://www.hotscripts.com" target="_blank">http://www.hotscripts.com</a>
