IIS Consolidation

We are beginning the process of consolidating our IIS 4 and 5 websites to IIS
6. Need advice on how best to accomplish the following on the old servers:

1.) Determine what sites are installed

2.) How those sites are being accessed(website alias)

3.) Determine how active the sites are(hits, etc.), if at all

4.) And finally, the cleanest way to migrate them over to IIS 6
