The first thing you need to do is turn off friendly http errors in your
browser, so we can get the real error message instead of a standard 500
internal server error.

Hey~
You most likely have a jacked up IWAM username and password... My first
guess is that after turning off HTTP friendly you will get Server
Application Error..
Quick test, go to the default Web site's properties, home directory,
Application protection and set this to Low (IIS Process)... Try hitting the
page - if this works, then it is a problem with COM+ ...
Also, you can access htm files as well as they are always served from
inetinfo...

Logon failure: user account restriction. Possible reasons
are blank passwords not allowed, logon hour restrictions,
or a policy restriction has been enforced.
What should I do?
In IIS, Anonymous auth is checker, IUSR_Computer is the
username, and IIS is controlling the password. The
IUSR_Computer name is in the Computer Management > Users
> IUSR_Computer name is added to the Guests group, the
user can't change the password, and the account never
expires.

Hey ~
You should open the Local Security Policy and find out if the user has log
on locally permissions. This is the only thing I can think of...
You could also remove it from Guests just to make sure there isn't any
inherited permissions that have been placed on Guest.
Also, are you on a domain? If yes, there are quite a few things that a
domain administrator can do to break things through security policies (like,
set the account to only be able to logon to bogus machines)...
