That error occurs when the user being impersonated by IIS (either the
configured Anonymous User, or the authenticated user if you are forcing
authentication) does not have NTFS permissions to the resource requested in
the URL
If you are unsure about what account is accessing what resource and being
denied, then you can turn on Object Auditing or use Filemon from
www.sysinternals.com (filter for Access Denied events)
